The idea for Care Team Connect stemmed from a personal and challenging experience in founder Ben Albert’s life. After his grandfather suffered a stroke, Albert witnessed firsthand the fragmented and uncoordinated nature of healthcare, which inspired him to find a better way to manage patient care across different providers. The company was founded to tackle this exact problem. Care Team Connect developed a web-based platform designed to help create and manage treatment plans for patients as they moved between hospitals, doctors, and home care. The goal was to ensure everyone involved in a patient's journey—from medical staff to family members—could collaborate effectively, improving outcomes and reducing costly hospital readmissions. By 2013, the Evanston, Illinois-based company had gained significant traction. Their focus on population health management and creating a transparent, accountable system for patient care caught the eye of a major industry player. A pivotal moment came in November 2013 when The Advisory Board Company announced it was acquiring Care Team Connect. The acquisition was a strategic move to integrate Care Team Connect’s platform into the Advisory Board's existing Crimson analytics suite. This combination created a powerful new tool, Crimson Care Management, aimed at helping providers deliver better value-based care, marking a successful exit and validation of the founders' vision.
